Looking for back pain relief near you that lasts longer than the last appointment? At Underground Gym in Red Bank, we coach adults over 40 to get strong again after back pain: the hip hinge, the squat, and kettlebell work done right, so your back stops taking the load for your hips and legs. No guesswork. No 'no pain, no gain.' Just coached strength, built around the back you have now.

Can strength training help back pain?

Cochrane review of 249 exercise trials: exercise therapy can help people living with chronic low back pain. At Underground Gym, we build on that idea with coached hip hinge, squat, kettlebell and carry work, adjusted to the back you have now.

Should I see a physical therapist or a personal trainer for back pain?

Start with your doctor or physical therapist for new pain, an injury, numbness or weakness, or any warning signs, so you get a diagnosis and treatment first. Once you are cleared, a coach at Underground Gym can help you build the strength and habits that keep your back working long term.

NIAMS Living With Back Pain lists warning signs to watch for, including pain that does not improve, numbness or weakness in the legs, and changes in bowel or bladder function. If you notice any of these, talk with your doctor promptly.

What helps lower back pain long term?

Long-term support for lower back pain usually comes from a mix of staying active, building hip and leg strength so your back stops doing all the work, and keeping up habits you can sustain, not from a single session alone.

That means regular movement, coached strength work, walking, sleep, and a plan you can stick with week after week, always with your doctor or physical therapist in the loop when pain is new or changing.

What exercises help lower back pain?

The fundamentals we coach are the hip hinge, the squat, the carry, and core work, practiced with light loads, good technique, and a coach watching your form, so strength builds around your back instead of through it.

We start with the hinge pattern, then the squat, then carries such as the farmer carry, and core work such as dead bugs and plank variations. Loads stay light until the pattern is solid, and every movement is scaled to the back you have now.

Start here - two routines from our own channel

Low back pain routine

Tight, achy low back? This is the routine Sergei gives members for lower-back tightness and discomfort - the same starting point we use in the studio.

Mid back pain routine

Stiff between the shoulder blades from desk years? Sergei demonstrates the mid-back exercises we use to open it back up.

Go gently, and stop if a movement causes radiating pain or numbness. If you have radiating pain, numbness, or an uncleared recent surgery, talk to us or your doctor first.

What exercises should you avoid with lower back pain?

Avoid exercises that load your back in a rounded, uncontrolled position, force it through pain, or twist under load, especially loaded sit-ups, heavy rounded deadlifts, and any movement that sends pain down a leg. When in doubt, ask your doctor or physical therapist first, and let a coach scale the movement to you.

Are kettlebells safe if you have back pain?

Kettlebells can be a useful tool when the technique is coached and the load is right for you, but they are not safe or unsafe in themselves; coaching, load, and your history matter. In our coaching opinion, a kettlebell swing or deadlift pattern should only be added once you can hinge well without pain, and only under a coach's eye.

Is walking good for lower back pain?

WalkBack trial: a progressive walking program helped people with recurrent low back pain go longer before their next episode, according to research published in The Lancet in 2024. Walking is a simple, low-impact way to stay active, and we often build it into a member's week alongside coached strength work.

How does the Back Pain & Movement Support program work?

InBody scan results help us see where you are starting from, then the program moves through three stages: assessment, corrective and core work, and progressive strength training.

REVIVE (Pravilo) is the mobility piece of the program, used alongside coached strength work. Your assessment looks at how you move, your history, and any guidance from your doctor or physical therapist, then corrective and core work prepares you for progressive strength training with the hinge, squat, carry, and core fundamentals.

How much does it cost?

Options start at $45 a week, week-to-week, with no long contract, and your free consultation will confirm which option fits your goals and schedule.

What happens at the free consultation?

At your free consultation, we talk through your history, your goals, and any guidance from your doctor or physical therapist, look at how you move, and explain how coaching at Underground Gym would work for you, with no obligation to join.

What our members say

"If you're in search of experienced, knowledgeable, compassionate personal trainer, look no farther. Sergei is the person you're looking for. I have problems with my back, burning and tingling in my feet and hands. Visited multiple doctors, tried acupuncture and no relief, until I meth with Sergei and he developed a personal program made specifically for me. Exercises, stretching and strengthening on Pravilo machine, which releases lots of tension. I'm a nurse and it is not easy for me to trust anyone with my health, but I definitely 100 percent trust Sergei. This is life changing experience. I feel so much better, I able to sleep at night without pain, and go with my life as a healthy normal person. Sergei is an excellent trainer. He will listen to all of your concerns and explain what he is doing every step of the way. Also Lana at the Underground gym is exceptional nutritionist, and she can help with nutrition plan, lose weight and stay healthy, like she helped me. I would highly recommend Sergei and Lana to anyone include my friends and family."
